目的 探討以pokemon蛋白判斷胃上部癌手術(shù)切緣的安全距離。
方法 對60例胃上部癌,在腫瘤中心以及距上下切緣1.0、1.5、2.0、2.5、3.0、3.5、4.0、4.5及5.0cm處分別取材。采用Western blot法檢測pokemon蛋白的表達情況。
結(jié)果 60例胃癌組織中,pokemon蛋白表達陽性38例,表達陽性率為63.3%。與腫瘤
中心處比較,上下切緣距離為1.0~2.0cm處組織中pokemon蛋白表達陽性率的差異均無統(tǒng)計學意義(P≥0.003),
而2.5~5.0cm處組織中pokemon蛋白的表達陽性率均較低(P<0.003)。上下切緣距離達3.5 cm及以上處組織中pokemon蛋白的表達陽性率均為0。上下切緣對應(yīng)位置pokemon蛋白的表達陽性率比較差異均無統(tǒng)計學意義(P>0.05)。
結(jié)論 以pokemon蛋白的表達情況作為對胃上部癌組織切緣的研究依據(jù),結(jié)果提示3.5cm及以外的組織pokemon蛋白的表達與正常組織無差異,可能對外科手術(shù)切緣的確定具有一定指導意義。
