【摘 要】 目的 比較克氏針張力帶、重建鋼板聯(lián)合張力帶及鷹嘴解剖鋼板固定尺骨上段合并鷹嘴骨折3種方式的生物力學(xué)穩(wěn)定性,為臨床選擇內(nèi)固定提供理論依據(jù)。 方法 8個(gè)自愿捐獻(xiàn)的新鮮成人尸體肘關(guān)節(jié)標(biāo)本,均為男性;年齡26~43歲,平均34.8歲。于尺骨上段及尺骨鷹嘴分別截骨制作尺骨上段合并鷹嘴骨折模型。每個(gè)標(biāo)本分別采用克氏針張力帶(A組)、重建鋼板聯(lián)合張力帶(B組)及鷹嘴解剖鋼板(C組)3種方式對(duì)骨折端固定。采用生物力學(xué)測試系統(tǒng)進(jìn)行單軸壓縮試驗(yàn),記錄載荷-位移曲線,內(nèi)固定系統(tǒng)的穩(wěn)定性采用骨折端壓縮位移為2 mm時(shí)所加的載荷值進(jìn)行評(píng)價(jià)。 結(jié)果 實(shí)驗(yàn)過程中未出現(xiàn)克氏針退出、鋼板螺釘斷裂、標(biāo)本破壞,標(biāo)本與夾具固定無松動(dòng)。3組標(biāo)本均表現(xiàn)為位移隨載荷增加而逐漸增長,但B、C組的載荷-位移曲線斜率明顯高于A組。當(dāng)骨折端壓縮位移為2 mm時(shí),A、B、C組的所加載荷值分別為(218.6 ± 66.9)、(560.3 ± 116.1)、(577.2 ± 137.6)N,B、C組所加載荷值均顯著高于A組,差異有統(tǒng)計(jì)學(xué)意義(P lt; 0.05);B、C組間差異無統(tǒng)計(jì)學(xué)意義(t=0.305,P=0.763)。 結(jié)論 尺骨上段合并鷹嘴骨折多為不穩(wěn)定骨折,重建鋼板聯(lián)合張力帶、鷹嘴解剖鋼板固定均能滿足要求,臨床上可根據(jù)患者情況合理選用??耸厢槒埩Ч潭ú粔蚶喂?,臨床上應(yīng)避免單獨(dú)使用。
