【摘要】 目的 探討125I粒子植入治療中晚期胰腺癌的臨床并發(fā)癥及相關(guān)護(hù)理措施?！》椒ā』仡櫡治?006年10月-2010年4月121例行125I粒子植入治療的胰腺癌患者的臨床治療及護(hù)理，采取積極有效的護(hù)理措施，預(yù)防及處理并發(fā)癥，促進(jìn)患者康復(fù)?！〗Y(jié)果 121例患者接受放射性125I粒子植入治療后，7例出現(xiàn)胰瘺，13例出現(xiàn)胃腸道反應(yīng)，經(jīng)對(duì)癥處理和精心護(hù)理后均治愈。 結(jié)論 125I粒子組織間植入近距離治療中晚期胰腺癌近期療效好、安全、副反應(yīng)少，良好的護(hù)理對(duì)改善中晚期胰腺癌患者預(yù)后具有重要意義。【Abstract】 Objective To observe the complications in patients with middle and advanced pancreatic carcinoma due to treatment of 125I particle implantation, and the investigate the proper nursing methods. Methods The clinical data including the therapy and nursing methods for the complications in 121 patients with middle and advanced pancreatic carcinoma due to treatment of 125I particle implantation from October 2006 to October 2010 were retrospectively analyzed. The treatment and care for complication were analyzed. Results In the 121 patients who had been treated by 125I particle, pancreatic fistula occurred in seven, and severe gastrointestinal upset was found in 13. The patients with the complications recovered after postoperative treatment and nursing. Conclusion 125I particle implantation for patients with middle and advanced pancreatic carcinoma is effective and safe with a few side reactions; proper nursing is important to improve the prognosis.
目的 建立人中分化結(jié)腸腺癌細(xì)胞裸鼠皮下移植瘤模型，以探討碘-125（125I）粒子治療中分化結(jié)腸腺癌的有效性。方法 將人中分化結(jié)腸腺癌細(xì)胞（SW480）種植到雄性BALB/c-nu/nu裸小鼠皮下，建立動(dòng)物模型，用隨機(jī)抽簽法隨機(jī)將荷瘤裸小鼠分成對(duì)照組和實(shí)驗(yàn)組（每組24只）,對(duì)照組植入1顆空白粒子，實(shí)驗(yàn)組植入1顆劑量為1.48×107 Bq的125I粒子，觀察粒子植入后瘤體生長情況，分別于植入后第7、14、21及28天處死實(shí)驗(yàn)組與對(duì)照組荷瘤裸小鼠，每個(gè)時(shí)相6只裸小鼠。剝?nèi)×鲶w，分別用于免疫組化SP法測(cè)定增殖細(xì)胞核抗原(PCNA)的表達(dá)，原位末端標(biāo)記(TUNEL)法檢測(cè)腫瘤細(xì)胞凋亡情況。結(jié)果 隨著放射時(shí)間的延長，從第10天起實(shí)驗(yàn)組腫瘤體積明顯小于對(duì)照組(Plt;0.05)，從第14天開始PCNA標(biāo)記指數(shù)表達(dá)顯著低于對(duì)照組(Plt;0.05)，從第21天開始凋亡指數(shù)顯著高于對(duì)照組(Plt;0.05)。結(jié)論 125I粒子通過持續(xù)低劑量輻射后可以下調(diào)PCNA的表達(dá)，誘導(dǎo)人中分化結(jié)腸腺癌細(xì)胞凋亡增加，是其抑制人中分化結(jié)腸腺癌增殖可能的生物學(xué)機(jī)理之一。
